You are viewing the web site for the original version of the DHQ. The latest version for the U.S. is the DHQ III; however, DHQ III does not yet have a Canadian version. The Canadian version of DHQ II is still available.
The files for the DHQ paper-based forms were updated in February 2007 with changes to the Today's Date field to include the years 2007-2011. Sample questionnaire forms, coding instructions, and the questionnaire data dictionary associated with each scanning method and for keypunch have been provided.
If you are beginning a new study and plan to use paper-based forms to capture the responses, download the DHQ1.2007 files.
